Imperial College Healthcare Trust enlisted Methods to help manage a project to issue a single discharge notification for patients and provide timely summaries for GPs, within 24 hours of patient discharge.

Solution

Methods has helped to deliver key project milestones, each resulting in substantial savings and rewards. We helped to design a new system for sending section notifications from wards to Social Services and introduced a system which provided staff with auto-populated patient details via the hospital patient administration system. A standardised template was also introduced in conjunction with an NHS London information sharing programme.

Results

Our activities have led to a 62% reduction in paper-based work, created a single system for the electronic transfer of information to local authorities, and minimised errors and duplicate information.

Imperial College Healthcare NHS Trust is committed to improving the quality and timeliness of discharge information to community partners, primarily GP’s as part of the Commissioning for Quality and Innovation (CQUIN). Methods change management team has been integral in achieving the CQUIN and improving discharge information.
